Community care loses patients in the gaps: a missed referral, a letter to an old address, a follow-up nobody owned. The clinical work is fine. The coordination is not.
Ruth is a GP who ran a deprived-area practice and built a spreadsheet that cut missed follow-ups by a third. The company started there.
